These Terms of Service govern the purchase and provision of services and products offered by Axiom Healthspan, including telehealth services, in person medical services, and shipped products.

By scheduling services, purchasing products, or otherwise engaging with Axiom Healthspan, you agree to these Terms of Service.

1. Medical Services

Axiom Healthspan provides healthcare services through licensed providers. Medical services may include telehealth consultations, in person visits, evaluations, and treatment recommendations.

Medical services are provided only after appropriate consent and medical evaluation.

2. Telehealth Services

Telehealth services involve the use of electronic communications to deliver healthcare remotely. Telehealth is not appropriate for all conditions and is not intended for medical emergencies.

By participating in telehealth services, you acknowledge the limitations and risks associated with remote care and agree to the Telehealth Informed Consent and Disclosure.

3. No Guarantees

Medical outcomes vary by individual. Axiom Healthspan makes no guarantees regarding treatment outcomes, results, or effectiveness of services or products.

4. Prescriptions and Products

Prescription products are dispensed only pursuant to a valid medical evaluation and prescription by a licensed provider.

Once a prescription product has been dispensed or shipped, it cannot be returned or refunded except as required by law.

5. Payments

Payment is required prior to the provision of services or shipment of products unless otherwise stated in writing. Axiom Healthspan does not accept health insurance and does not submit claims to insurance providers. All services and products are provided on a self pay basis, and fees are the sole responsibility of the patient or customer. By providing payment information, you authorize Axiom Healthspan to charge applicable fees.

6. Cancellations and No Shows

Appointment cancellations, rescheduling, and no show policies are governed by the Cancellation Policy. Failure to comply with cancellation terms may result in fees.

7. Refunds and Returns

Refunds and returns for products are governed by the Refund and Returns Policy. Prescription products are generally non refundable.

8. Shipping

Shipping terms, timelines, and delivery conditions are governed by the Shipping Policy. Delivery dates are estimates and not guaranteed.

9. Compliance With Laws

You agree to comply with all applicable laws and regulations related to healthcare services, prescriptions, and use of products.

10. Limitation of Liability

To the fullest extent permitted by law, Axiom Healthspan shall not be liable for indirect, incidental, special, or consequential damages arising out of or relating to services or products. Nothing in these Terms limits liability for gross negligence or willful misconduct where such limitation is prohibited by law.

11. Indemnification

You agree to indemnify and hold harmless Axiom Healthspan from claims arising from misuse of products, failure to follow medical instructions, or violation of these Terms.

12. Governing Law

These Terms of Service are governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles.

13. Arbitration

Any dispute, claim, or controversy arising out of or relating to these Terms of Service, the services provided, or products purchased shall be resolved through binding arbitration administered by the American Arbitration Association in accordance with its Commercial Arbitration Rules. Arbitration shall take place in Pennsylvania, unless otherwise required by law. You agree that disputes will be resolved on an individual basis and waive any right to participate in a class or representative action, except where prohibited by law.

14. Modifications

Axiom Healthspan may modify these Terms of Service at any time. Continued use of services constitutes acceptance of the updated Terms.
