These Terms of Service govern the purchase and provision of services and products offered by Axiom Healthspan, including telehealth services, in-person medical services, and shipped products. By scheduling services, purchasing products, or otherwise engaging with Axiom Healthspan, you agree to these Terms.
Axiom Healthspan provides healthcare services through licensed providers. Medical services may include telehealth consultations, in-person visits, evaluations, and treatment recommendations. Services are provided only after appropriate consent and medical evaluation.
Telehealth services involve the use of electronic communications to deliver healthcare remotely. Telehealth is not appropriate for all conditions and is not intended for medical emergencies. By participating, you acknowledge the limitations and risks of remote care and agree to our Telehealth Informed Consent.
Medical outcomes vary by individual. Axiom Healthspan makes no guarantees regarding treatment outcomes, results, or effectiveness of services or products.
Prescription products are dispensed only pursuant to a valid medical evaluation and prescription by a licensed provider. Once a prescription product has been dispensed or shipped, it cannot be returned or refunded except as required by law.
Payment is required prior to the provision of services or shipment of products. Axiom Healthspan does not accept health insurance and does not submit claims to insurance providers. All services are provided on a self-pay basis. By providing payment information, you authorize Axiom Healthspan to charge applicable fees.
Appointment cancellations, rescheduling, and no-show policies are governed by the Cancellation Policy. Failure to comply may result in fees.
Refunds and returns are governed by the Refund and Returns Policy. Prescription products are generally non-refundable.
Shipping terms and delivery conditions are governed by the Shipping Policy. Delivery dates are estimates and not guaranteed.
To the fullest extent permitted by law, Axiom Healthspan shall not be liable for indirect, incidental, special, or consequential damages arising out of or relating to services or products.
These Terms are governed by the laws of the Commonwealth of Pennsylvania. Any dispute shall be resolved through binding arbitration administered by the American Arbitration Association in Pennsylvania. Disputes will be resolved on an individual basis.
Axiom Healthspan may modify these Terms at any time. Continued use of services constitutes acceptance of the updated Terms.
