This Cancellation Policy applies to all services, programs, and orders offered by Axiom IV Hydration Therapy, Inc., doing business as Axiom Healthspan (“Axiom Healthspan,” “we,” “us,” or “our”), and purchased through axiomhealthspan.com or associated Axiom Healthspan systems.
1. No Refunds for Cancellations
Axiom Healthspan does not offer refunds for cancelled services. All sales are final.
Once a service, consultation, medical program, or treatment plan is purchased, payment is non refundable regardless of whether the service has been used, partially used, not used, or voluntarily discontinued.
2. Medication and Peptide Orders
Axiom Healthspan provides customized medical protocols and prescription grade products. Prescription medications and peptide products are final sale once submitted for clinical review or pharmacy fulfillment. Orders cannot be cancelled once they have been submitted for clinical review, provider authorization, or pharmacy processing.
Final Sale:
Once a prescription medication or peptide order is placed, it cannot be cancelled, recalled, or refunded.
Regulatory Restrictions:
Federal and state regulations prohibit the return, reuse, or resale of prescription medications after they have been dispensed or shipped.
3. Appointment and Consultation Policy
Refunds are not provided for cancelled appointments. Appointment rescheduling may be permitted under the conditions outlined below.
Rescheduling:
Appointments may be rescheduled at no additional charge when at least twenty four hours’ notice is provided.
Late Cancellation and No Show Fee:
Appointments cancelled or rescheduled less than twenty four hours prior to the scheduled start time, as well as missed appointments without notice, may be subject to a non refundable fee of $50.
4. Telehealth Medical Ineligibility
If a patient is determined to be medically ineligible for a medication program during a required telehealth consultation, a refund for the product cost may be issued at Axiom Healthspan’s discretion.
Credit card processing fees incurred at the time of purchase are non refundable and will be deducted from any approved refund amount.
5. Policy Scope
This Cancellation Policy applies solely to appointment scheduling, telehealth visits, in person medical services, and related cancellations or rescheduling. Refunds and returns for products are governed separately by the Refund and Returns Policy.
