Risks: Potential interruptions, unauthorized access, technical difficulties, or limitations related to the absence of an in-person physical examination.
Telehealth may not be appropriate for all medical conditions. Certain diagnoses, treatments, or clinical situations require in-person evaluation or testing. Your provider will inform you if an in-person visit is recommended.
Telehealth services are not intended for medical emergencies. If you experience a medical emergency, call 911 or seek immediate emergency care at your nearest emergency facility.
Reasonable safeguards are used to protect your personal and medical information during telehealth communications. However, electronic communications carry inherent security risks that cannot be completely eliminated. We use encrypted, HIPAA-compliant platforms where possible.
Telehealth services are provided only in states where the healthcare provider is licensed or otherwise legally authorized to practice. Your provider will confirm eligibility prior to your consultation.
By participating in telehealth services, you voluntarily consent to receive healthcare via telehealth and acknowledge the risks, limitations, and conditions described in this document. You may withdraw this consent at any time by notifying your provider.
