HIPAA Notice of Privacy Practices

Overview

This Notice of Privacy Practices describes how Axiom IV Hydration Therapy, Inc., doing business as Axiom Healthspan, may use and disclose your protected health information ("PHI") and how you can access this information. We are required by law to maintain the privacy of your PHI, provide you with this Notice, and follow the terms of the Notice currently in effect.

1. How We May Use and Disclose Your PHI

Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare treatment. This includes sharing information with other healthcare providers involved in your care.

Payment: We may use and disclose your PHI to obtain payment for services rendered, including billing and collection activities.

Healthcare Operations: We may use and disclose your PHI for our internal healthcare operations, including quality assurance, staff training, and legal compliance activities.

2. Other Permitted Uses and Disclosures

  • As required by federal, state, or local law
  • To public health authorities for disease prevention or control
  • To law enforcement in limited circumstances
  • In response to a court order or subpoena
  • To avert a serious threat to health or safety

3. Your Rights Regarding Your PHI

Right to Access: You have the right to request a copy of your medical records and other PHI maintained by Axiom Healthspan.

Right to Amend: You have the right to request that we amend your PHI if you believe it is inaccurate or incomplete.

Right to an Accounting: You have the right to request an accounting of certain disclosures we have made of your PHI.

Right to Restrict: You have the right to request restrictions on certain uses and disclosures of your PHI.

Right to Confidential Communications: You have the right to request that we communicate with you in a specific way or at a specific location.

Right to a Copy of This Notice: You have the right to receive a copy of this Notice.

4. Our Duties

We are required by law to maintain the privacy of your PHI, provide you with this Notice, and abide by the terms of this Notice. We reserve the right to change our privacy practices and the terms of this Notice. Changes will apply to PHI we already hold as well as PHI we receive in the future.

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with Axiom Healthspan or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
