This Refund and Returns Policy applies to all products, services, programs, and consultations provided by Axiom IV Hydration Therapy, Inc., doing business as Axiom Healthspan (“Axiom Healthspan,” “we,” “us,” or “our”), and purchased through axiomhealthspan.com or associated Axiom Healthspan systems.
1. Final Sale and No Refunds
All sales are final. Axiom Healthspan does not offer refunds, returns, or exchanges for products once a purchase is completed, except as expressly stated in this policy or required by law.
This policy reflects the medical nature of our services and the specialized handling required for prescription grade products.
2. Prescription Medications and Peptide Products
All prescription medication and peptide orders are non refundable once submitted for clinical review, provider authorization, or pharmacy processing.
Regulatory Compliance:
Federal and state regulations prohibit the return, restocking, reuse, or resale of prescription medications and compounded products once they have been dispensed or shipped.
No Cancellations:
Medication orders cannot be cancelled once submitted for clinical review, provider authorization, or pharmacy processing.
3. Services and Appointments
Refunds are not provided for cancelled services, programs, or appointments. Appointment cancellation and rescheduling terms are governed by the Cancellation Policy.
Late Cancellation and No Show Fee:
Appointments cancelled or rescheduled with less than twenty four hours’ notice, as well as missed appointments without notice, may be subject to a non refundable fee of $50.
4. Medical Ineligibility Exception
A refund for the product cost may be issued at Axiom Healthspan’s discretion if a patient is determined to be medically ineligible for a specific program during a required telehealth consultation conducted by a licensed provider.
Credit card processing fees incurred at the time of purchase are non refundable and will be deducted from any approved refund amount.
This exception does not apply once a medication order has been approved, processed by the pharmacy, or shipped.
5. Damaged or Incorrect Shipments
Customers must notify Axiom Healthspan of any damaged shipment or fulfillment error within forty eight hours of confirmed delivery.
Claims are evaluated on a case by case basis. Approved claims may result in a replacement at our discretion. Refunds are not issued for shipping related issues.
Shipping fees are non refundable, and replacements do not extend or reset any applicable refund or cancellation timelines.
